Correspondence Re : Expansion of Cancer Care and Control in Countries of Low and Middle Income: A Call to Action
The Global Task Force on Expanded Access to Cancer Care and Control in Developing Countries (GTF-CC) sounds a clarion call: However, by failing to recognize the strictures in high income countries, the authors foreclosed on the principal gateway for the global expansion of cancer care and control – International Cancer Research and Technology Transfer (ICRETT).Mid-20th century, ICRETT in sub-Saharan Africa enabled the promulgation and ratification of the U.S. National Cancer Act (1971), which ushered in an era of tremendous investments in cancer care and control. However, in the 20th century, ‘structural inequality' was embedded in the system of international (North-South) trade and the political-industrial complexes in the North (the U.S. notably) established monopolies and bent the cost-curve upwards – to the point of diminishing returns. The rising global cancer burden against soaring costs in cancer care and control speaks to profound strictures – power asymmetries and moral hazards – in the global value chain. In the 21st century, the telecommunications-computer-transportation-logistics revolution is enabling global sourcing – i.e. symmetrical (“flat”) North-South trade relations and improved relational governance – and improving efficiencies in the global value chain. To paraphrase Paul Krugman, “the only important structural obstacles to world prosperity are the obsolete doctrines that clutter the minds of men”. In this vein, we reframe the discourse of the GTF.CCC from ‘structural inequality' to the ‘structuration of equity': “structuration” connotes the interrelation of parts in an organized whole and underscores the growing importance of relational governance and; “equity” connotes the moral contingencies and social sanctions of value creation and distribution
