Is the Dual Burden of Over- and Under-nutrition a Concern for Poor Households in Ethiopia, India, Peru and Vietnam?(¿Es el doble problema de sobrepeso y desnutrición una preocupación para los hogares pobres en Etiopía, India y Vietnam?)
El rápido desarrollo económico y la creciente urbanización han llevado a un dramático incremento del sobrepeso en los adultos de los países de bajos ingresos. Paradójicamente, el sobrepeso suele coexistir con la persistente desnutrición entre los miembros de un mismo hogar. El objetivo de este estudio fue evaluar si la desnutrición infantil combinada con el sobrepeso materno es una preocupación de salud de los hogares muy pobres; así como si la residencia urbana está asociada con el fenómeno. La ocurrencia de baja talla para la edad y/o desnutrición infantil y el sobrepeso materno dentro del hogar fue estudiado en cohortes de 2000 niños de 4.5 y 5.5 años de edad en cada uno de los paises pertenecientes al estudio (Etiopía, India, Vietnam y Perú). La regresión logística multivariada fue usada para estimar el efecto de la residencia urbana, controlando factores potenciales confusos. La prevalencia de baja talla para la edad del niño -desnutrición infantil- combinada con el sobrepeso materno fue baja en Etiopía, India y Vietnam, pero fue hasta de 14% en el Perú. La relación de niños de bajo peso y madres con sobrepeso es poco común en los cuatro países. Pero en la India, la residencia urbana incrementa significativamente la probabilidad de ese doble problema en el hogar. En el Perú y en Vietnam, vivir en un vecindario urbano reduce ligeramente la posibilidad de la coexistencia de baja talla para la edad y el sobrepeso materno. En conclusión, la doble carga de sobrepeso y desnutrición parece ser una preocupación de salud solo para los hogares pobres en el Perú. Por ello, se requieren intervenciones específicas para mejorar el estado nutricional de los distintos miembros del hogar individualmente -madres, diferente que hijos- así como promover un estilo de vida saludable.
