Neuordnung der ambulanten Onkologie: Gutachten im Auftrag des Berufsverbandes der niedergelassenen Hämatologen und Onkologen e.V., der Deutschen Gesellschaft für Hämatologie und Onkologie e.V. und der Deutschen Krebsgesellschaft e.V.
Die Neuordnung der ambulanten spezialisierten fachärztlichen Versorgung steht auf der politischen Agenda. Gerade im Versorgungsbereich der Onkologie gibt es seit langem anhaltende Diskussionen über die vorherrschenden unterschiedlichen Betriebsformen, verschiedenen Zulassungsbedingungen mit unterschiedlichen Qualifikations- und Qualitätsanforderungen sowie Vergütungssysteme der Leistungserbringer. Das vorliegende Papier, das durch den Lehrstuhl für Medizinmanagement der Universität Duisburg-Essen im Auftrag des Bundesverbands der Niedergelassenen Hämatologen und Onkologen e.V. (BNHO), der Deutschen Krebsgesellschaft (DKG) und der Deutschen Gesellschaft für Hämatologie und Onkologie e.V. (DGHO) erstellt wurde, schlägt eine Neuordnung der organisatorischen Rahmenbedingungen der ambulanten onkologischen Versorgung im System der gesetzlichen Krankenversicherung vor. Übergeordnetes Ziel ist die Entwicklung eines einheitlichen Ordnungs- und Organisationsrahmens für die ambulante Onkologie, der eine effiziente, bedarfsgerechte, qualitätsorientierte, innovationsgerechte und flächendeckende onkologische Versorgung für alle Patienten, unabhängig vom behandelnden Leistungserbringer, gewährleistet. Um eine nachhaltige Diskussion über die Praktikabilität und Praxistauglichkeit der erarbeiteten Vorschläge zwischen allen Akteuren in der ambulanten Onkologie zu initiieren, wird hier die Entwicklung eines einheitlichen Ordnungsrahmens in einem ersten Schritt exemplarisch am Beispiel der medikamentösen Tumortherapie als einem gut definierbaren Bereich der ambulanten Onkologie entwickelt. Resultat des durch das vorliegende Papier angestoßenen Diskussionsprozesses sollte aber die Erarbeitung eines einheitlichen Rahmens für die gesamte onkologische ambulante Versorgung sein, wobei das Konzept grundsätzlich auch Pilotcharakter für weitere Bereiche der ambulanten spezialisierten fachärztlichen Versorgung haben kann.
