Bürgerversicherung und Pauschalprämie : Stellungnahme zu den gesundheitspolitischen Empfehlungen der "Rürup-Kommission"

Der Bericht der Regierungskommission für die Nachhaltigkeit in der Finanzierung der Sozialen Sicherungssysteme ("Rürup-Kommission") stellt zwei Modelle zur Finanzierung der Krankenversicherung gegenüber: die sog. Bürgerversicherung und das Modell der pauschalen Gesundheitsprämien mit staatlichen Transferzahlungen. Das Modell der Bürgerversicherung sieht prinzipiell vor, dass sich alle Bürger in der gesetzlichen Krankenversicherung im Rahmen einer Grundsicherung pflichtversichern müssen. Die Bemessungsgrundlage stellen Arbeitseinkommen und Einkommen aus Vermögen dar. Privaten Versicherungen bleibt nur die Möglichkeit, über die Grundversicherung hinausgehende Risiken zu versichern. Im Modell der Pauschalprämie zahlt jeder gesetzlich Versicherte den gleichen Betrag, und die bisher im System integrierte Umverteilungsfunktion wird durch ein staatliches Transfersystem übernommen. Zudem wird die paritätische Finanzierung durch Arbeitgeber und Arbeitnehmer aufgegeben. Die genaue Rolle der privaten Versicherungen bleibt im Bericht offen. In der öffentlichen Diskussion wird zumeist auf die kurzfristigen Beschäftigungs-, Wachstums- und Verteilungseffekte dieser Modelle abgehoben. Jedoch sollte eine Reform des Gesundheitssystems nicht primär auf diese Effekte abzielen, sondern auf die Vermeidung von ökonomischen Fehlanreizen und die grundsätzliche Steigerung seiner Effizienz. Im Folgenden werden beide im Entwurf vorgestellten Modelle anhand von sechs Kriterien bewertet: (1) Anreizmechanismen, (2) Stabilität bzw. Nachhaltigkeit, (3) Beschäftigungs- und Wachstumsfreundlichkeit, (4) soziale Gerechtigkeit, (5) administrativer Aufwand und (6) Anpassungsfähigkeit. Dabei wird sich zeigen, dass das Pauschalprämienmodell dem der Bürgerversicherung überlegen scheint. Gleichwohl sind beide nicht in der Lage, das Effizienzproblem im Gesundheitssystem umfassend zu lösen. Hierfür wären weiter gehende Strukturreformen nötig.
